xG Health Solutions, powered by Geisinger , and Casenet, announced today a strategic alliance which will deliver
evidence-based assessment and care plan content to the health plan market. Through this partnership, xG's Intelligent Care Management™ evidence-based content will be available in Casenet's TruCare GuidedCare product. The clinical content is based on nationally-recognized practice guidelines and standards of care, including assessments and care plans for 45 conditions that are most commonly addressed in care management programs.
TruCare GuidedCare and its branching logic are embedded within the TruCare platform, immediately providing care managers with the right tools at the right time to maximize member interaction and outcomes. The solution integrates xG Intelligent Care Management™ content with TruCare client-configured workflows to enable use of content that is aligned to each client's requirements; including evidence-based assessments that can be used out of the box or configured to meet business and regulatory needs.

TruCare GuidedCare features 45 condition-specific assessments, which are based on nationally recognized guidelines. The assessments include a series of questions that identify potential opportunities to improve member outcomes or achieve healthcare goals. There are both condition-specific and condition independent (i.e., social, behavioral, and environmental) components to an assessment. Care team members are automatically presented with evidence-based and member-specific care plans including problems, goals and interventions based on assessment responses. All the information captured in the assessments and care plans is stored seamlessly within TruCare.
