TruCare’s Case Management module supports a multi-disciplinary approach to the management of members and participants. All care team participants would have access to a single, integrated plan of care, while each would have their own assignments. Individual problems, goals, interventions and outcomes can be documented by each member of the care team with all authorized participants able to view the single plan of care and coordinate to ensure that all are working collaboratively throughout the cycle of care management.

 

Based on client-defined permission profiles, authorized care managers would have access to a member’s assessment and can update evidence-based assessment plans and interventions for both medical and behavioral diagnosis. Care plan problems, goals, interventions, barriers, progress toward goals, priority and ultimately, outcomes can all be updated and edited by authorized care team participants. Care managers can also add attachments and notes via TruCare Structured Notes.

The TruCare Disease Management module enables quick design and deployment of personalized disease management programs. TruCare Disease Management (DM) integrates new and existing data from multiple sources including member and provider inputs. Client-defined assessments, care plans, correspondence and TruCare Structured Note templates provide the tools to effectively manage all disease management programs on a single, integrated platform.

 

Client-defined disease management programs can be customized using the non-technical configuration tools provided with the TruCare administrative console. Assessments, care plans, TruCare Structured Notes, correspondence templates and automated identification rules can be tailored to meet the needs of each specific DM program, such as diabetes, asthma and other chronic conditions or targeted diagnosis. Client-defined workflow and routing rules can also be configured to ensure that identified cases, tasks and interventions are delivered to the appropriate team.
