Home and Community-Based Services Enable Members To Live At Home Cost-Effectively

 

Healthcare is rapidly evolving to meet the needs of an aging population who experience multiple co-morbid conditions and are living longer. Enhanced coordination of care, including home and community-based services (HCBS), is essential to reducing medical costs and delivering improved population health.

 

It is estimated that “the Medicaid program can provide HCBS to three people for the cost of serving one person in a nursing home."1 As a result, home and community-based services are being utilized more heavily in traditional treatment plans to enable the management of members outside a conventional healthcare facility while proactively managing chronic conditions. When the appropriate use of home and community-based services are incorporated into a member’s care, the plan’s members are able to stay in a more familiar, independent setting and take a more proactive role in their care while allowing treatment to be administered in a cost effective manner.
