Opportunities in the Medical Home
The medical home health care model is going mainstream. What has been a successful model of care delivery within pediatric Medicaid populations is gaining interest with commercial health plans and employer groups. Driving some of the interest is the increasing acknowledgment that a lack of care coordination within the U.S. health care system is perpetuating fragmentation of care, rising costs and poor health outcomes.
