BEDFORD, MA.,- Casenet LLC, a population health technology company, has developed innovative technology that allows Medicare Advantage plans to seamlessly incorporate social determinants of health into the care management continuum, keeping members healthy and independent. TruCare, Casenet's population health technology platform, enables plans to identify members in need of non-medical support and match them to community providers. Services can include Meals on Wheels, non-emergency transportation, housekeeping, personal care services, in-home palliative care and more.
The Centers for Medicare and Medicaid Services will give Medicare Advantage plans more discretion to offer supplemental benefits in 2019 in an effort to keep members healthy and independent. According to CMS, nearly 270 Medicare Advantage plans will provide an estimated 1.5 million enrollees new types of supplemental benefits in 2019 such as adult day care services, in-home services, and support services.

The TruCare population health platform is flexible, enabling plans to load community providers into the system. Care managers can then select the appropriate resources and build care plans that include non-medical services. The entire care team has visibility into the services that have been allocated to each member which enables care teams to effectively coordinate care.
